Injuries, safety concerns, and/or near-misses occurring on campus or while participating in university business should be reported within 24 hours.


Report any injuries, illness, or exposures sustained during work duties here, whether you are a full-time or part-time employee.

Use this form to report a safety, near miss, non-life-threatening hazard, or hazardous event or condition.

Non-employee students and visitors report all other injuries or illnesses sustained during non-work activities, such as during class, recreation, RCO activities, or personal time.
